When you are accepted into the Charter Oak Health Plan, you will be asked to enroll in one of our participating health insurers - Aetna Better Health, AmeriChoice by Unitedhealthcare or Community Health Network of Connecticut –to coordinate your health care benefits.
Coverage includes preventive care, emergency room and hospital visits, primary care and specialist physicians, pharmacy, behavioral health services and prescription medications. In order to choose from the member health insurers, locate participating physicians, hospitals and community health centers, and get a copy of a member handbook with detailed information on the plan, please visit the individual websites or call their customer service numbers listed below.
